Case 114

I have frontal fibrosing alopecia. I needed to switch doctors recently (for a variety of reasons). The new doctor removed me from my methotrexate & hydroxychloroquine and is only giving steroid injections & dutasteride. I have now developed dark patches in my underarms and groin. It’s everywhere! I’m so depressed! Is dutasteride causing this? How do I get rid of this? It’s so embarrassing!

Case 115

My scalp condition started out as a small scaly horn-like area around a bundle of hair in 2017/2018. A dermatology specialist in late 2018 felt that I had an “actinic keratosis” and the lesion was treated with liquid nitrogen.

In February 2019, my scalp was itchy and a biopsy was done. I was told that the condition was benign and stress-related, so I looked for a shampoo that calmed the itchiness and worked on my stress management. Ketoconazole was given to me but it made my itchiness worse and was not continued due to discomfort and increased symptoms.

By March 2024, there was thinning of hair noted. The area was very scaly and scabby. A new biopsy was taken. While waiting for the results, I was prescribed a scalp oil daily, followed with a rinse with T/sal shampoo in the morning.

My biopsy result showed folliculitis decalvans. My doctor mentioned I could use 3-months of antibiotics but this could be hard on the gut. I mentioned that I already have gut issues, so we decided to alternate scalp oil with clobetasol every other night and check back in 2 months. By May 2024 (just 2 months later), I had lost a lot of hair - dramatically more hair. I am considering starting doxycycline but I live in a very sunny area. Now, I see on the clobetasol information sheet that hair loss can be a side effect too!

Questions:

1) What is my correct diagnosis?

2) Can clobetasol have caused cicatricial alopecia?

3) Should I stop Clobetasol?

4) How often does doxycycline cause sun reactions? What is the statistic?

5) Did prior liquid nitrogen or ketoconazole cause this particular scarring alopecia?
